Online Services - Individual account creation checklist
Use the checklist below to create your Individual Online Services account. You will need to know the following information to successfully create your account.
If you don't have the filing information (found on your tax return) required to create your account, you can use Form DTF-505, Authorization for Release of Photocopies of Tax Returns and/or Tax Information, to request a copy of your return. For your protection, we cannot provide you with filing information over the phone.
We suggest you print this checklist for your records.
Note: If the Tax Department sends you a letter with a five-digit PIN, use the PIN to create your Individual Online Services account. You will not need to report the following information.

Have at hand a New York State income tax return (Form IT-201 or IT-203) filed for one of the past five years. You will need to provide the following filing information:
the taxable income reported on your return: _______________________, and
the requested refund amount from your return: _______________________, or
the amount owed you reported on your return: _______________________.
the 10-digit assessment ID number from a tax bill (open or closed): _______________________.
Note: If you have not filed a New York State income tax return for one of the past five years and have never received a bill from the Tax Department, you will still be able to create an Individual Online Services account.

Have at hand your contact information (name, address, email, and phone number).
Choose a username (8-60 characters consisting of only letters, numbers, and these special characters: .@_-): _______________________.
Be sure to save your username in a secure place. We cannot reset usernames.
Choose a password (8-20 characters): _______________________.
